What cardiac screening is for

Cardiac screening looks for cardiovascular problems and risk factors before they cause symptoms. Much of the damage that leads to a heart attack builds silently over years, and the first noticeable event can be a serious one, so the purpose of screening is to identify risk while there is still time to act on it.

Screening is not a single test. It combines a history of symptoms and family background, a physical examination, blood pressure measurement, blood tests for cholesterol and glucose, and one or more cardiac investigations chosen according to the individual’s risk profile.

What the assessment usually covers

A cardiovascular risk assessment considers factors you cannot change, such as age, sex and family history, alongside those you can, including blood pressure, cholesterol, blood glucose, body weight, smoking and physical activity. These are combined to estimate overall risk rather than judged in isolation.

Depending on that assessment, further testing may include an electrocardiogram, an assessment of the heart under exertion, an ultrasound scan of the heart, or imaging of the coronary arteries.

Who screening tends to suit

Screening is generally more informative for people with a family history of premature heart disease, those with known risk factors such as high blood pressure, raised cholesterol or diabetes, people who smoke, and those returning to demanding physical activity after a long gap.

It is also reasonable for anyone who has noticed symptoms on exertion — chest tightness, unusual breathlessness, or palpitations — though symptoms should prompt assessment rather than routine screening.

Understanding the results

A normal screening result reflects the tests performed at that point in time; it does not guarantee future cardiovascular health, and risk factors can change. Equally, an abnormal finding on one test is a reason for further assessment rather than a diagnosis in itself.
